Questionnaire PDF Print E-mail

No generalized survey can do justice to determining eligibility for the potential tax benefits related to disability. ***   A personal confidential review is required, and must be extremely detailed.   At DTS we don’t create a form to fill out, we simply suggest that if you answer yes to any of the following, you should call or email us to set up a free initial consultation.

Does Any Of The Following Apply To You Or a Family Member?

For the parents of a child with a disability:

My child has a disability that impairs his or her ability to perform one or more regular daily activity (Examples: walking, speaking, hearing, dressing, eating, or coping with social or work situations).

We have a child with a disability and one parent devotes significant time to the care of that child, such that they are not fully employed.

My child requires therapy or special medical devices for a visible disability, or a non-visible disability such as a learning disability.

My child is falling behind in school or social skills as a result of disability.

My child receives specialized assistance at school as a result of disability.

For adult persons with a disability:

There are significant costs related to my disability.
